OVARIAN CYST ... OVARIAN CANCER

The ovarian cyst is a usually benign tumor. But when it turns malignant, it is a cancer of the ovary. If the cysts are common, fortunately cancerous tumors are rare.

Ovarian cysts may occur at any age

The ovarian cysts can occur at any age and usually at puberty and before menopause. They are often discovered during a routine pelvic examination or pelvic ultrasound. Sometimes, they appear as small pain lower abdomen (often on one side and felt during intercourse), irregular periods accompanied by bleeding (bleeding outside of menstruation) or urinary discomfort because if thecyst is large, it tends to compress the bladder and cause a feeling of heaviness. An ovarian cyst may also be suspected when the start of pregnancy is to wait (it can block ovulation). A path that becomes hoarse and hair that is growing can also turn on the trail of a tumor.

Overall, there are two main types of cysts:

  • Functional cysts, supported by oral contraceptives (especially the mini-pill), stimulant drugs and ovarian pregnancy (first trimester). These are the most common and usually disappear spontaneously within a few months (between 6 weeks and 3 months). They tend to occur regularly, with each cycle. Indeed, at each cycle, a follicle evolves to give an egg. If the cyst, the follicle grows abnormally without releasing an egg. It is for this reason that a cyst can delay pregnancy.
  •  Organic cysts also called "true cysts", contain some very unexpected in a ovary, such as fatty tissue, bone, cartilage, teeth, hair, etc.. It speaks of dermoid cyst when tissues are similar to the skin, serous cyst when it is filled with liquid, mucoid cyst (fat) or even if it contains tissue dysembryoma hair, hair. Most often it is necessary to remove such cystsbecause they can break the abdomen.

Ovarian cyst: how to confirm the diagnosis?

Ultrasound is the first diagnostic tool. Performed through an abdominal or vaginal procedure, it can determine the size of the cyst, shape, location, mobility, its uni-or bilateral, consistency homogeneous or heterogeneous, liquid or solid, the thickness of the wall or even the possible existence of intracystic vegetations. In case of functional cyst, ultrasound shows a tumor fluid, thin-walled. Evolution is often spontaneous, additional tests are carried out some weeks later, where appropriate, that is to say if the cyst has not disappeared. Still under ultrasound, it is possible to puncture. The analysis of the sample thus collected can direct the diagnosis towards a functional cyst or organic. In the same context, a Doppler allow visualization of the vasculature of the cyst. If an ovarian cancer is suspected, it performs a scan of the abdomen and pelvis.

What are the treatments of ovarian cyst?

In the rare cases where a functional cyst does not disappear spontaneously, treatment may be prescribed. This is most often a pill estrogen-progestin dose.If a complication is suspected, it usually decides to remove it by laparoscopy (after a tiny incision, a small tube with a camera and sampling tools are inserted, allowing surgery "closed abdomen"). If it is a malignant tumor is suspected, the patient is immediately directed to a specialized oncology center.

What about ovarian cancer?

It is fortunately much rarer than cysts. The risk is slightly higher after menopause and in women (whatever their age) who have personal or family history of breast and / or ovarian cancer. These patients routinely receive a particularly thorough consultation. In case of suspicion of ovarian cancer, the procedure is done by laparotomy (incision of the abdominal wall). During surgery, a sample is analyzed to confirm the nature of cancerous cyst. If this is the case, we proceed to a removal of the uterus, fallopian tubes and ovaries. This surgery is often supplemented by chemotherapy.

Cysts: beware of complications

Whether organic or functional, some cysts may cause mild complications is the case when making a twist, they break or become infected. Some symptoms are therefore an emergency requiring immediate surgery: severe pain on one side, nausea, vomiting and pallor. Infection and rupture are also accompanied by a fever troll.

Key facts about the cysts

During the first trimester of pregnancy, cysts are common and, again, most often regress spontaneously. In all cases, avoid removing them because they grow at the expense of the corpus luteum, and thus contribute to the production of progesterone, a hormone essential for normal pregnancy.
